Our Annual Roselea FC End of Season Gala Day Four aside competition is on again.

This year we invite teams from our GHFA Mixed comp and Walking Football to join us. Each team will be playing games for approximately 3 hours during the day, including finals.

PLEASE READ ALL THE INSTRUCTIONS BELOW TO ENSURE A VALID TEAM ENTRY.

All teams must a have a Team Coordinator to ensure they are at the correct field at the correct time.

All players must wear full club uniform, including boots and shin pads.

Form a team with your winter team mates or from other teams within or outside Roselea FC.

Teams of 4 or 6 are recommended.

Please complete the questions with each team entry below including:
-All Players Full Name
-All players FFA Number or enter GUEST player for players not registered to play the 2019 winter season.
-Team Coordinator Full Name, Mobile and email address.
- Choose a team name.

Teams will not be entered into a competition unless all details are completed correctly as this effects your insurance coverage.

ALL GUEST PLAYERS (not registered for 2019 winter football)
- will have $5 day membership including insurance covered by Roselea FC.
- must enter DOB, email address in the questions below.
- will be lent a Roselea FC jersey for the day, please wear arkd blue or black socks and shorts.
- require a FNSW Non-Registered Player Consent & Indemnity Form to be completed, signed and returned to vpwomens@roseleafc.com. This can be downloaded at:

GUEST PLAYERS/TEAMS registered to play winter football with another club in 2019 will require a letter of release to play on the day, stating each player name and FFA Number. I will send this to GHFA/NWSWF so they are covered for insurance as this is a FNSW Sanctioned event.

We reserve the right to adjust age grouping to form viable competitions.
